Cannabis Businesses - Final Plan of Record Submission

Welcome! This page contains all required submission forms for final plans of record for cannabis business license applicants. These are only to be completed after you receive notification from the Office of Cannabis Management (OCM) that you’ve advanced to preliminarily approved status.

If you are an applicant for a lower-potency hemp edible business license, you can find guidance and required forms on the General Licensing Process webpage under "Resources for Applying for a Lower-Potency Hemp Edible Business License."

Once preliminarily approved, cannabis business license applicants must submit detailed final plans of record for their cannabis business. These forms document finalized site and operational details and must be submitted to OCM for approval before a pre-licensure inspection can be scheduled and a license issued. OCM has issued Guidance Memo GM-2025-01 for cannabis business license applicants with important information on how to successfully complete your final plans of record. Review this document before submitting your final plans of record.

How to Submit

Access the required fillable online forms using the links provided below.
Complete each form. You will receive a PDF copy of the completed form via email from no-reply@webmerge.me. Save this file where you can easily upload it to Accela in Step 4 below. The email containing the file also has a link you
Register your site in Accela after you have received a preliminary approval email from OCM, contacted your local government for zoning approval and your physical site is ready to be inspected.

To access site registration, sign in to your Accela account and select Create Application on the Accela homepage. Select Site registration from the list of available applications and the radio button before selecting Save and continue.

For detailed step-by-step instructions for submitting a site registration, please review Appendix A of the Qualified Applicant Guide to Obtaining a License.
Upload all PDF copies of the final plans of record you received by email to your Accela account on the second step (site information) of the site registration page. Please also upload any files from your final plans of record that were larger than 10MB and were unable to be uploaded on the online forms. If you filled out the change of ownership form, you must upload your updated capitalization table as well.

Important: Failure to upload your completed forms and required files to Accela will result in your submission being marked as incomplete.

Required Final Plans of Record (All Cannabis Business License Applicants)

All cannabis business license applicants must complete the following forms:

- If you have previously finalized this form and received a PDF of your completed Site, Security, and Operations FPOR, you may submit that document to OCM for review.
- If you have an application in progress using the original form, you may use your save link to access it and continue or make changes. You can use it to either transfer your form details to the new form or submit that version by 4 p.m. on June 26. After 4 p.m. on June 26, the prior form will be discontinued, and we can no longer guarantee your progress will be retained.
- Questions have changed with this update. If you are copying over details from a prior Site, Security, and Operations FPOR to the new form, please read each question carefully.
- Only the most recent save link will work, and your progress will expire after 30 days.

If you are a cannabis delivery service, cannabis transporter, or a cannabis microbusiness, cannabis mezzobusiness, or medical cannabis combination business that will transport products, you must complete the following:

1. Inspection certificates to be included in the Site, Security, and Operations Final Plan of Record, including all certifications from an independent third-party industrial hygienist* or professional engineer that approve:
  - Electrical systems
  - Gas systems
  - Fire suppression systems
  - Exhaust systems
  - Plans for safe storage and disposal of hazardous substances, including volatile chemicals
  - If you intend to use processes that do not require certification from an independent third-party industrial hygienist or professional engineer, please provide a report from a qualified independent third party confirming that such certification is not required.
