How to Participate as a Health Care Practitioner

Who can certify patients?

  • Minnesota-licensed doctor of medicine.
  • Minnesota-licensed physician assistant acting within the scope of an authorized practice.
  • Minnesota-licensed advanced practice registered nurse who has the primary responsibility for the care and treatment of the qualifying medical condition of a person diagnosed with a qualifying condition.

Practitioner enrollment details

icon of person on laptop, with a talk bubble with a checkmark in itBefore you can certify a patient, you must register in the Medical Cannabis Registry. That process includes creating a new user account (with a login and password) and completing the Health Care Practitioner Registration Form.

When you create your account, you will need your:

  • Medical license number (not your NPI)
  • DEA number
  • Primary clinic address information

To help protect against identity theft and to ensure the integrity of the program, someone from the Office of Medical Cannabis will contact you to verify your identity. You will receive an email notification when your account is approved.

Once your account is approved, you can certify patients.

How to certify your patient

  • Conduct a thorough exam.
  • Review your patient's medical records, diagnostic tests, and laboratory results to confirm diagnosis.
  • Develop an ongoing treatment plan for your patient's qualifying medical condition.

Log in to the Medical Cannabis Registry to complete the certification process. Note: To be eligible to participate in the Minnesota Medical Cannabis Program, a patient must be a Minnesota resident.

Registry Login

Click the "Certify Patient” under the Quick Links tab on your Practitioner home page.

  • Enter your patient's personal and contact information, and their qualifying medical condition(s).

Once you certify your patient, they will receive an email from the Office of Cannabis Management with instructions on completing their enrollment application.

In certifying that your patient has a qualifying condition, you are also attesting to your availability to manage their medical condition on an ongoing basis.

Patient recertification and re-enrollment details

Patients who are enrolled in the Medical Cannabis Program must have their qualifying condition(s) recertified and complete an application to re-enroll every three years.

  • Registered patients are allowed to have another participating health care practitioner certify their condition.
  • Participating health care practitioners can recertify patients up to 90 days before the patient's enrollment in the program expires.
