Cannabis Businesses - Final Plan of Record Submission
Welcome! This page contains all required submission forms for final plans of record for cannabis business license applicants. These are only to be completed after you receive notification from the Office of Cannabis Management (OCM) that you’ve advanced to preliminarily approved status.
If you are a lower-potency hemp edible business license applicant, you can find guidance and required forms on the General Licensing Process webpage under "Resources for Applying for a Lower-Potency Hemp Edible Business License."
Once preliminarily approved, cannabis business license applicants must submit detailed final plans of record for their cannabis business. These forms document finalized site and operational details and must be submitted and approved before a pre-licensure inspection can be scheduled and a license issued. OCM has issued Guidance Memo GM-2025-01 for cannabis business license applicants with important information on how to successfully complete your final plans of record. It is important to review this document before submitting your final plans of record.
How to Submit
- Access the required fillable online forms using the links provided below.
- Complete each form. A copy will be emailed to you as a PDF.
- Register your site in Accela, even if already entered in your application. To access site registration, log into your Accela account and select Create Application on the Accela homepage, which will automatically route you to the site registration page.
- Upload all final plans of record PDFs from your email to your Accela account on the second step (site information) of the site registration page. Please also upload any file uploads from your final plans of record that were larger than 10MB and unable to be uploaded on the online forms. If you filled out the change of ownership form, you must upload your updated capitalization table as well.
Important: Failure to upload your completed forms and required file uploads to Accela will result in your submission being marked as incomplete.
Required Final Plans of Record (All Cannabis Business License Applicants)
All cannabis business license applicants must complete the following forms:
- Site, Security, and Operations Final Plans of Record - Cannabis
- Inventory Control and Diversion Prevention Standard Operating Procedure (SOP) Final Plans of Record
- Quality Assurance Standard Operating Procedure (SOP) Final Plans of Record
- Accounting and Tax Compliance Standard Operating Procedure (SOP) Final Plans of Record

Businesses Engaging in Transportation or Delivery
If you are a cannabis delivery service, cannabis transporter, or a cannabis microbusiness, cannabis mezzobusiness, or medical cannabis combination business that will transport products you need to complete the following:
Making Business Changes (if applicable)
If you need to make any of the following business changes, you will need to work closely with the OCM, including receiving prior approval in some instances.
- Change of business ownership and control
- Change of business name
- Change of general business information
