According to Minn. R. 2500.1105, licensees must notify the MBCE of any name or address change within 30 days. Failure to notify the MBCE may create difficulties receiving renewal, continuing education or other important notices and is considered a violation of the Practice Act.
Name Change: Licensees must be licensed under their full legal name. Name changes require official legal documentation; such as an original/certified marriage certificate, divorce decree, or court order authorizing the name change. Due to the requirement of an original/certified documentation, a name change must be processed by paper/mail. Complete and print the Name Change Request form and mail it to the MBCE along with the official legal documentation. Your original/certified documents will be returned to you by USPS.
Address Change: Licensees may update their address, phone and email at any time by logging into their online account. MBCE must have a current email that goes directly to the licensee (not a clinic or assistant) for official communications, such as continuing education and renewal notices. MBCE does not share or publicly post email addresses of licensees.
