Out-of-State EMS Credential Verification

The Minnesota Office of Emergency Medical Services (OEMS) provides credential verification for individuals who hold or have previously held a Minnesota EMS credential and are seeking licensure, certification, or credentialing in another state.

Many states require verification of an applicant's credential history directly from the state where the credential was issued. If another state requires OEMS to complete a verification form on your behalf, you must submit an Out-of-State Credential Verification Request with the required documentation.

How to Request Credential Verification

Step 1: Obtain the Required Verification Form

Contact the EMS licensing or credentialing authority in the state where you are applying to determine its verification requirements.

Obtain the appropriate verification form and complete all sections designated for the applicant, including any required signatures.

Step 2: Complete the General Authorization and Release Form

OEMS requires a completed and signed General Authorization and Release Form before processing your request.

The authorization must identify the state licensing or credentialing agency that will receive your information. Complete all required fields, including the receiving state agency's information, and sign and date the form before uploading it.

Step 3: Submit Your Request

Complete the online Out-of-State Credential Verification Request and upload both required documents:

  • The verification form provided by the receiving state, with all applicant sections completed.
  • Your completed and signed General Authorization and Release Form.

You will also need your Minnesota OEMS provider number and the name of the state requesting verification.

What Happens After Submission?

OEMS reviews requests in the order received. Once the required documentation is received, OEMS will review your Minnesota credential information and complete the applicable sections of the receiving state's verification form.

Completed verifications will be provided to the receiving state licensing or credentialing authority in accordance with its submission requirements and applicable data privacy laws.

Please note:

  • Requests missing required documentation cannot be processed.
  • Incomplete or incorrectly prepared forms may be returned for correction.
  • OEMS completes only those sections designated for the licensing or credentialing authority.
  • Applicants are responsible for ensuring they meet the application and verification requirements established by the receiving state.
  • Submitting a verification request does not constitute an application for licensure or certification in another state.

Public Credential Verification

If you only need to confirm the current status of a Minnesota EMS credential, you may use the public OEMS License/Credential Lookup.

The public credential lookup is separate from the formal out-of-state verification request process. If another state requires a completed verification form from OEMS, follow the instructions above.
