Ambulance services wishing to replace an ambulance, must complete the Ambulance Replacement Vehicle Application form in the eLicense Portal. There is a requirement to report the following information:
- Year
- Make
- Model
- VIN
- Unit Number
- Chassis Remount (Yes/No)
- Box Manufacturer
Upon completion of the application, you will receive an email confirmation allowing the vehicle to be placed into service until the EMS Specialist can complete an inspection.
