MNSTAR Elite
MNSTAR Elite is Minnesota’s system for submitting and reviewing electronic patient care reports (ePCRs) for pre-hospital emergency medical services. MNSTAR Elite supports EMS reporting, quality improvement, and system-level oversight. Ambulance services are required to submit ePCRs to MNSTAR Elite within 24 hours of call completion. Authorized hospital users may access submitted patient care reports through the OEMS Hospital Hub.

If MNSTAR Elite is unavailable or cannot be accessed, ambulance services may use the MNSTAR Field Run Form as a backup submission option.

PCR Data Quick Guide (NEMSIS & PWW)
The PCR Data Quick Guide is a reference document developed by NEMSIS that explains key data elements and documentation standards used in electronic Patient Care Reports (ePCRs). It provides practical guidance on how to correctly record patient care information according to the NEMSIS v3.5 standard and supports consistent, high-quality documentation across EMS agencies.

Minnesota EMS Quick Report (Paper Form)
This paper form is intended for use when electronic documentation is not available, such as during computer or system outages. The form is used to provide patient care information to the receiving facility at the time of patient transfer and must be left with the receiving facility before the crew departs.
This form does not replace the requirement to submit an electronic patient care report (ePCR) and does not capture all Minnesota-required data elements. Ambulance services must submit an ePCR to MNSTAR Elite within 24 hours of call completion. Hospitals may access completed patient care reports through the OEMS Hospital Hub.
