MNSTAR Minnesota State Ambulance Reporting

The Minnesota State Ambulance Reporting System (MNSTAR), also known as ImageTrend Elite, is the electronic patient care reporting system used by the Office of Emergency Medical Services (OEMS) and licensed ambulance services to document and monitor prehospital emergency care. OEMS uses this data to protect and improve the health and safety of Minnesotans.

Through MNSTAR/Elite, Minnesota-licensed ambulance services report and review prehospital data for quality improvement, system evaluation, and administrative purposes. This data-driven partnership between OEMS and ambulance services supports ongoing improvement in emergency care across the state.

This paper form is intended for use when electronic documentation is unavailable, such as during system outages. It provides patient care information to the receiving facility at the time of transfer and must be left with the receiving facility before the crew departs.

This form does not replace the requirement to submit an electronic patient care report (ePCR) and does not capture all Minnesota-required data elements. Ambulance services must submit an ePCR to MNSTAR Elite within 24 hours of call completion. Hospitals may access completed reports through the OEMS Hospital Hub.
