Rural Health Transformation Program: Ambulance Service Treatment-in-Place (TIP) Pilot

The Rural Health Transformation Program: Ambulance Service Treatment-in-Place Pilot supports eligible rural Minnesota ambulance services in implementing Treatment-in-Place (TIP) programs. These programs help improve access to healthcare, reduce unnecessary ambulance transports to emergency departments, and support a pilot reimbursement model for qualifying ambulance services.
Treatment-in-Place refers to a clinically meaningful assessment and/or intervention performed by licensed EMS personnel in response to a 911 call, in accordance with the agency’s Medical Director-approved protocols, that results in a documented medical decision not to transport the patient by ambulance.

2026 Application Cycle
The 2026 application period opens July 6, 2026. Applications are due August 17, 2026, at 4:00 p.m. Central Time. Grant agreements are anticipated to begin October 1, 2026.
Purpose and Outcome of the Grant Opportunity
This grant opportunity is funded in collaboration with the Minnesota Department of Health’s Office of Rural Health and Primary Care as part of the federally funded Rural Health Transformation Program.
The Office of Emergency Medical Services Rural Health Transformation Program will support access to healthcare by creating a pilot for rural ambulance services to provide Treatment-in-Place. This RFP supports enhancing access to rural healthcare while creating a pilot reimbursement model for qualifying ambulance services to improve economic feasibility.
Grant outcomes include:
- Increase healthcare access within rural Minnesota.
- Establish Treatment-in-Place ambulance service protocols.
- Improve health outcomes for rural Minnesotans.
- Reduce ambulance transports to the emergency department.
- Reduce repeat 911 calls.
- Provide reimbursement for ambulance services providing treatment under TIP protocols.
Eligibility Criteria
To be eligible for a grant under this section, an ambulance service must primarily provide ambulance services outside the metropolitan counties of Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, or Washington.
Available Funding
Funding will be allocated through a competitive process. There is an estimated $9,218,100 available for this grant program. OEMS anticipates awarding approximately 15 to 40 grants. The estimated award minimum is $35,000, and the estimated award maximum is $750,000.
There is no match requirement for this grant.
What the Grant Covers
Eligible expenses include costs associated with providing a qualifying Treatment-in-Place ambulance response. To qualify, the ambulance response must result in a documented medical decision not to transport the patient by ambulance. Patient care reports for ambulance responses will be reviewed by OEMS to ensure they meet eligibility for reimbursement. All other expenses are ineligible.
Grant Period
The grant period is anticipated to begin October 1, 2026, and end August 31, 2027.
No work on grant activities may begin until a fully executed grant contract agreement is in place and the State’s Authorized Representative has notified the grantee that work may start.

Applications Requesting $50,000 or More
The following exhibits are only required for grant applications requesting $50,000 or more:
- Exhibit C: Capacity Responses. Attachment.
- Exhibit D: Certification that the entity is not suspended or debarred by the State of Minnesota or the federal government. Attachment.
- Exhibit E: Evidence of Good Standing. Attachment.
- Exhibit F: Nonprofit grantee documents, as applicable. Attachment.
- Exhibit G: For-Profit Certification Disclosure and required documents, as applicable. Attachment.
- Exhibit H: Certification that no current principals have been convicted of a felony financial crime in the last ten years, including the list of principals. Attachment.
Incomplete applications will be rejected and not evaluated. Applications must include all required application materials, including attachments. Do not provide any materials that are not requested in this RFP, as such materials will not be considered or evaluated.
OEMS reserves the right to reject any application that does not meet these requirements.
By submitting an application, each applicant warrants that the information provided is true, correct, and reliable for purposes of evaluation for potential grant award. The submission of inaccurate or misleading information may be grounds for disqualification from the award, as well as subject the applicant to suspension or debarment proceedings and other remedies available by law.
All costs incurred in responding to this RFP will be borne by the applicant.

Community Grant Reviewers
The Minnesota Office of Emergency Medical Services (OEMS) is seeking community members to serve as volunteer reviewers for the Treatment-in-Place Grant Program.
Reviewers will evaluate grant applications remotely and provide scores and comments that help inform funding recommendations. OEMS is especially interested in individuals with lived or professional experience in emergency medical services or a related field. Reviewer orientation will be provided. Reviews are expected to take place from late August through early September 2026. Selected reviewers must be Minnesota residents, have access to a computer and internet connection, and must not be applying for a Treatment-in-Place Grant.
