List of Board Actions
Click List of Board Actions to view a list of all disciplinary and corrective actions issued by the Board. This document:
- is organized by year and contains all public disciplinary and corrective actions issued by the Board from 1991 to the present.
- can be searched for specific information by typing "Ctrl F" while viewing the document and entering the letters, words, dates, or numbers you are looking for in the search box that appears in the upper right corner of the document.
Go to Public License Lookup to view the complete public document for the disciplinary or corrective action listed:
- type in the licensee's name or license number (numbers only)
- click search and click on the licensee's name
- scroll down to "Public Actions" and click the download icon
Please note: License numbers in the List of Board Actions document are preceded by a two letter code which corresponds to the individual's license type as follows:
- LS=LSW
- LG=LGSW
- LI=LISW
- LC=LICSW
