February 5, 2024 - The Department has established that Appellant violated licensing standards applicable to AFC and HCBS license holders and that he provided false and misleading information to the Department. As a result, the Department has established reasonable cause exists to revoke Appellant’s licenses, and Appellant has not proven by a preponderance of the evidence that he was in compliance with all the statutes and rules at issue. The Commissioner of the Department (Commissioner) should AFFIRM the Revocation Order.
