December 29, 2022 - The Administrative Law Judge concludes that the Department correctly determined that Appellant failed to comply with three terms of the Order of Conditional License and correctly determined that Appellant committed 15 licensing violations. Further, Appellant failed to demonstrate that it complied with the laws and rules of the Department. Therefore, the Administrative Law Judge recommends that the Commissioner of the Department of Human Services (Commissioner) AFFIRM the order revoking Appellant’s license to provide adult day services.
