MNSTAR Elite Run Form (ImageTrend Elite Field™ EMS)

The MNSTAR Elite Run Form is an offline reporting option intended for use when MNSTAR Elite cannot be accessed due to technical or operational limitations. The form allows EMS personnel to document and share essential patient care information with the receiving facility at the time of patient transfer.

Use of the MNSTAR Elite Run Form does not replace the requirement to submit a complete electronic patient care report (ePCR). All ambulance services must submit an ePCR to MNSTAR Elite within 24 hours of call completion once system access is available.
