Homeowner Insurance Reporting

Minnesota Statutes, sections 65A.28 and 65A.29 require insurers to submit certain types of data related to homeowners’ insurance on an annual basis. Below you will find links to webpages where you can submit the reporting templates for Minn. Stat. §§ 65A.28 and 65A.29.

The reporting templates for Minn. Stat. §§ 65A.28 and 65A.29 linked below contain a single worksheet on which data should be entered for a single company. If you wish to submit data for multiple companies, create a copy of the first worksheet within the template workbook for each additional company and rename each worksheet appropriately to signify which company’s data is contained on that worksheet.

Note that the template for Minn. Stat. § 65A.29 contains a limited number of rows for the table in which data should be entered. If the number of zip codes in which the company has data exceeds the number of rows in the template, please add additional rows to the table as necessary to account for the total number of zip codes that contain relevant data.

Additionally, you will find attestation forms linked below for Minn. Stat. §§ 65A.28 and 65A.29 reports. If you are submitting data for multiple companies, please add additional bullet points for the names of each company in the appropriate section of the attestation form.
