New Online Reporting Forms for Deaths and Serious Injuries are now available

Minnesota State Flag New Online Death and Serious Injury Reporting is now available.

Please note: The online webforms cannot be saved. Each report needs to be completed in one session. Enter the information and click the “Next” button, verify your information and click the “Submit” button. You will then have the option to print the form for your records.

Diagnosis and medication information may be entered directly into the online webform or faxed. If you choose to fax additional information to the Office, please fax it to 651-797-1950 along with the case number which will be generated and displayed after you submit each report.

Service providers are required to notify the OMHDD within 24 hours of the death or serious injury of a client. The OMHDD reviews reports of deaths and serious injuries of persons receiving services or treatment for mental illness, developmental disabilities, chemical dependency, or emotional disturbance.
