Death and Serious Injury reports can now be submitted online. Completion of the webform is the preferred method of submission to OMHDD. Please note that the online webforms cannot be saved. Each report needs to be completed in one session. Enter the information and click the “Next” button, verify your information, and click the “Submit” button. You will then have the option to print the form for your records. Diagnosis and medication information may be entered directly into the online webform or faxed. If you choose to fax additional information or reports to the OMHDD, please fax it to 651-797-1950 along with the case number displayed upon submission of the report when using the webform. The OMHDD can receive faxes twenty-four hours a day. (Please be advised that reporters also required to submit reports to DHS should continue to do so.)
