In Facility Transitions Centers

What is a Transitions Center?

Transition Centers are in-facility spaces where incarcerated people can work on their release plans in efforts to transition successfully into the community.  Our Transitions Coordinators, Case Workers and Release Planners provide referrals and connections to community-based service providers and other government offices to help people meet their release needs.  Some of the most common referrals:

  • Navigating child support, establishing paternity, parenting classes and support
  • Veteran's Services and assistance to establishing benefits
  • Substance use disorder treatment and community-based meetings
  • Mental health treatment and community-based meetings
  • Mentoring and peer supports
  • Understanding driving related license fines and fees
  • Financial literacy
