Program eligibility

 You qualify for MN ACT if you: 

  • Live in Minnesota.
  • Have a hearing loss, speech or physical disability that limits your use of a standard phone. 
  • Have a landline or wireless phone service, or have applied for phone service. 
  • Have family income within the following guidelines (effective October 1, 2023 – September 30, 2024) 
Family size: Income less than:
1 person $65,335
2 people $85,439
3 people $105,542
4 people $125,645

If there are more than four people in your home, please contact us for income guidelines. 

How to apply

  1. Fill out an application (TED applications are still good! New MN ACT applications will be available October 1, 2024):
  2. Make copies of required documents (see page 2 of the program brochure for details):   
    • Proof you live in Minnesota: Your driver's license or state identification card.
    • Proof of phone service: Your phone bill.
    • Proof of income: The first page of your tax return or bank statement showing direct deposits.
    • Proof of disability: Certification of disability form (in the application).
  3. Send your completed application and document copies to MN ACT by mail, email or fax.

    • Fax: 651-431-7587

A specialist will review your application. They will contact you to discuss your needs and what device(s) can improve your access to the phone. If you do not qualify, they will help you find other resources.
