Exemption from Reporting to the MN PMP

Dispensers meeting any of the following criteria are exempt from reporting to the MN PMP, but must complete and submit an Exemption Request form:
 

• Opioid Treatment Program pharmacy or facility that is unable to report due to data privacy measures in Federal Regulations 42 CFR part 2.

• Licensed hospital pharmacy that distributes controlled substances as defined in Minnesota Statutes 152.126, for inpatient hospital care only.

• Pharmacy or facility that solely distributes drugs to individuals through the use of an automated drug distribution system in accordance with Minnesota Statutes 151.58.

• Pharmacy or facility that never dispenses controlled substance (schedules II-V, butalbital and gabapentin) prescriptions. Be sure to review the list of Drugs that are Required to be Reported to the MN PMP.
