New/Change Application

Use the Pharmacy License Application to apply for a new pharmacy license, to notify the Board of an upcoming relocation, or if your pharmacy has had a change in ownership.

Renewal Applications and Instructions

The Board of Pharmacy recently migrated to an online licensing/registration platform. Renewals for pharmacies are now available

Minnesota’s licensing fees were revised effective 7/1/2023. 

All fees are non-refundable.

To avoid late fees, renewal applications, supporting documentation, and correct payment must be received as a complete packet by the Board, prior to the expiration date. Any payment received without an application will not be processed. Licenses are mailed to the facility’s physical address.

Remodel Application

If you are a RESIDENT pharmacy and plan to remodel,  complete the Pharmacy License Application, and submit it to the Board. Non-Resident pharmacies are not required to submit an application for remodel.

The remodel plan must be submitted at least 60 days before the proposed changes. Upon receipt the Board will review the plan for compliance with the required elements and provide a preliminary approval letter. With the submission include a blueprint of the current layout clearly indicating what is changing with the remodel, and a blueprint or diagram for the final layout indicating all required elements.

Period of Issuance:

July 1 - June 30 annually.


Length of Determination: On site inspections of pharmacies located within the state of Minnesota are required before license will be issued.
