License Verification

Written Verification of Minnesota Licensure

A license verification letter, provides written verification of licensure from the Minnesota Board of Occupational Therapy Practice to the state in which you are applying for a license. License Verification letters include: full name, date of birth, license number, issue date, expiration date, status and information regarding public discipline and corrective action. 

 

If you have an active or expired license, request and pay for an official verification through your online account. Log in through the "Account Login" and select "Online Services" in the upper right hand corner. All first time users must "Register to Access Site". Verification request are processed within three (3) to five (5) business days.
