A license verification letter gives written proof of licensure from the Minnesota Board of Occupational Therapy Practice to another state or jurisdiction.
 
It includes:
  • Full name
  • Date of birth
  • License number
  • Issue and expiration dates
  • Status
  • Public discipline and corrective action
Request a Verification:
Request a verification through your online account. Requests process within 3 to 5 business days.
  1. Log in to your online account
  2. Navigate to the "General" tab
  3. Select "License Verification"

Search for occupational therapy practitioners at no cost using the License Lookup tool. The Minnesota Board of Occupational Therapy Practice License Lookup tool is considered primary source. It is updated in real-time as changes occur within the system.
