If you require a duplicate license card, you may place an order by accessing your online account, selecting "Request Duplicate Card" in the license tile, and paying the duplicate card fee. If you require a duplicate copy of your wall certificate, please contact the Board office.
Ensure your mailing address is correct by accessing your online account through "
Account Login". Minnesota Statute §148.6428, requires all licensees to maintain current name, address, and primary employment information. All notices or other correspondence mailed to or served on a licensee at the licensee's address on file shall be considered as having been received by the licensee.
