- Early intervention medical services
- Schedule a Comprehensive evaluation with a provider listed in the MN Healthcare Program Provider Directory or by searching Minnesota Aging and Disability Resources. Search using the key words ‘Early Intensive Developmental and Behavioral Intervention’ or ‘EIDBI’ and then ‘CMDE assessments.’ The Comprehensive Multi-Disciplinary Evaluation (CMDE) provider will help determine eligibility and medical necessity for early intervention medical services.
- For a step by step guide to scheduling an evaluation, use the Pathway to Services Referral Tool or EIDBI Referral Form. The referral form is sent directly to the early intervention team at DHS. A member of the team will reach out to you and help you get connected to EIDBI services or other supports. The Referral Tool is also available in Lus Hmoob - Hmong, Af Soomaali - Somali and Español - Spanish.
- Contact your local county or tribal office to apply for health care coverage or to help identify potential services your child may be eligible for. When you call, ask about case management services and scheduling a MnCHOICES Assessment and Support Plan.
It is important to explore both medical and educational pathways to make sure your child gets everything they need. Some services, like Medical Assistance and Social Security, require a medical diagnosis to be eligible. An educational determination allows students to access special education services through the public school system.
