General Information

  • Application forms are fillable PDFs. You may save your application to your computer's hard drive to complete the application in more than one sitting.

  • Please read the application packet information carefully. You must meet all of the requirements listed at the time of application.

  • The appropriate fee must accompany your application. No credit cards and no faxed or e-mailed applications will be accepted. You can find fee information on the page specific to your profession.

    Active and Veteran Military Members: Some fees may be waived or reimbursed under various state and federal programs. See the Military page for details.

  • Transcripts must be official copies and be sent to the Board in their original, sealed envelope.

  • Applicant information is confidential under the Minnesota Data Practices Act. In order for a third party to inquiry about the status of an application, you will need to fill out and submit the following form:
