Transition from Toddler/Infant Intervention Services The service coordinator must help transition the child and family from the infant and toddler intervention services before the child's third birthday. If the child is eligible for special education, the coordinator must help with the transition to receiving those services. With the permission of the child's family, the transition services may include a meeting with the family, the local educational agency, and community-based providers to discuss future services the child may need. If the child is not eligible for special education services than the coordinator, with the parents' permission, should set up a meeting with the family, the lead agency, and community-based service providers to decide appropriate services the child may receive once the infant and toddler intervention services have ended.
