Certification forms

When you apply for Paid Leave, you will need to provide a certification document. For most types of leave, this will be a certification form. These forms verify your reason for taking leave. Each type of leave has its own form.

You only need to complete the form that matches the type of leave you are requesting. Your healthcare or service provider will also need to fill out part of the form and sign it.

All certification documents need to be signed. This is usually done by a healthcare or other service provider.

All forms are fillable PDFs. You can complete them electronically or print and fill them out by hand.

How to submit your form

You will upload your certification form when you apply for Paid Leave. If you’ve already applied, you can submit it later through your Paid Leave Account or by mail.

How certification works

Paid Leave certification forms are similar to Family and Medical Leave Act (FMLA) forms. Because Paid Leave covers more situations than FMLA, some leave that qualifies for Paid Leave may not qualify for FMLA.

  • Paid Leave accepts FMLA certification forms if they include all the required information
  • Employer-developed forms may be accepted case-by-case if they contain all required information. Paid Leave forms are recommended.
