Any licensing and processing fees are your responsibility and all fees are nonrefundable. Applications will not be considered complete until all required documents are received by the Board office.

Temporary permit fee = $25.00, required for the 6 month supervised traineeship
Exam = $50.00, required if applicant does not already have a passing score on the exam
The temporary permit request form and fee ($25) cannot be submitted to the Board prior to the receipt of the applicant's initial application and fees.
The Exam fee ($50) must be paid for each exam attempt. To retake the exam you must mail the exam fee ($50 - check or money order) to the Board office.
Foreign Educated applicants must apply for licensure by submitting a paper application and fee to the Board via mail.

Required supporting documentation will be accepted by the Board office independent of the application.
After the Board receives your initial application for licensure and fees you will receive an email with instructions on how to set up an online account or to log into an existing online account to view the current and ongoing updates to the status of your application.
