Toxicology Screening Information

If you are participating in HPSP because of a substance use disorder, toxicology screens are used as a tool for documenting your sobriety, as well as an adjunct to maintaining sobriety. Your Participation Agreement requires you to follow the Toxicology Instructions. To be successful in HPSP, we recommend you take the necessary time to become familiar with all the information and that you again review the instructions with your case manager Included in the Toxicology Instructions are the following documents:

Participant Signed Acknowledgement Form (Signed Acknowledgement form - participant completes and returns to HPSP)
Hennepin Health Care Account Generation Form (Participant completes and returns to HPSP)
Collection Site Protocols Agreement (Collection site completes, keeps original and returns a completed copy to HPSP)

Directions to HHC: Park at the main hospital ramp attached to the main hospital.  The address for that is: 615 South 6th Street, Mpls, MN 55415. Proceed to the skyway level to check in.  Cross the skyway, you will be on the 2nd floor of the purple building. Take elevators to 4th floor.  Look for Clinical Laboratories sign. Tell staffs at the front desk area, you are here for a HPSP collection. Important: We encourage timely communication with your case manager concerning any toxicology screen problems you encounter; for example, if you forgot to call the toxicology phone number, or if you collected your specimen after the 6:00pm deadline, and so on.

Directions for PETH: If you are asked to provide a PETH test, go to Hennepin Healthcare Clinic & Specialty Center Lab. There is a parking ramp beneath the building.  That address is 821 Park Ave. S, Mpls, MN 55415. Otherwise there are street parking available.   Proceed to the 2nd floor and check in at client services area.  Please identify yourself here for an unscheduled HPSP Peth collection.
