A license verification letter, also known as a license certification, or letter of good standing, provides written verification of licensure from the Minnesota Board of Physical Therapy. If you are applying for a license in another state or province, there is usually a requirement for license verification from other states where you have been licensed. The fee to have the Minnesota Board of Physical Therapy send a license verification to another licensing board is $25.00. You can request and pay for a license verification through your online account. New users may need to register with our online services website prior to submitting a license verification. Alternatively, a hard copy paper request form (paper form) is available and can be mailed to the MN Board of PT with a check in the amount of $25.00 made payable to the "MN Board of Physical Therapy".
Active licensees may request and pay ($25.00) for an official verification through online services. Please log in to your online services account and under the general category, click the option "License Verification" to request license verification online.
If you held a temporary permit in Minnesota prior to full licensure and need your temporary permit information verified to another state or province, please contact the Board at 612-627-5406 or physical.therapy@state.mn.us. Temporary permit information is not available through online services and is considered a special request.
Official license verification letters include: full name, date of birth, license number, issue date, expiration date, status and information regarding public discipline and corrective action. Verification requests are processed within three (3) to five (5) business days.
The license verification will be mailed to the address you enter on the recipient contact information page. Please confirm the mailing address with the jurisdiction prior to submitting your request.
