A license verification letter, also known as an affidavit, license certification, or letter of good standing, provides written verification of licensure from the Minnesota Board. If you are applying for a license in another state or province there is usually a requirement for license verification from other states where you have been licensed. The fee to have the Minnesota Board of Physical Therapy send a license verification to another licensing board is $25.00. You can request license verification and pay online via our online services website. New users may need to register with our online services website prior to submitting a license verification. Alternatively, a hard copy paper request form (paper form) is available and can be mailed to the MN Board of PT with a check in the amount of $25.00 written out to the "MN Board of Physical Therapy".
Active licensees may request and pay ($25.00) for an official verification online. Please log in to your online services license page and click "online services" in the upper right hand corner to request verification online.
If you held a temporary permit in Minnesota prior to full licensure and need your temporary permit information verified to another state or province, please contact the Board at 612-627-5406 or physical.therapy@state.mn.us. Temporary permit information is not available through online services and is considered a special request.
Official verification of licensure form Minnesota includes full name, date of birth, license number, issue date, expiration date, status and information regarding public discipline and corrective action. The process time for MN Board of Physical Therapy verification is 3-5 business days.
The verification will be mailed to the address you enter on the Recipient information page via the online services license page. Please confirm the address with the jurisdiction prior to submitting your request.
