INSTRUCTIONS FOR DIETITIAN OR NUTRITIONIST ONLINE LICENSE APPLICATION
All applications for a new Minnesota License must be completed using the online application. The board is no longer accepting paper applications.
- All licenses issued before November 1 will expire November 30 of issuing year. If licensed November 1 or after expire November 30 of the following year.
1. Read the laws that govern the practice of dietetics and nutrition in Minnesota.

Enter your name the way you want it to appear on your Official License Certificate. The name you enter must exactly match the name of the supporting documents, or documentation of formal name change must be submitted, i.e., marriage certificate, official court document (Photocopies only do not send original document).
After completing the Online Application, you will receive an email from the board that your application has been received and need to submit the required documentation:
If you are applying for a Licensed Dietitian:
- Application Basis - Current Registration with the Commission on Dietetic Registration (Preferred Method): You must submit a notarized copy of your current registration card from the Commission on Dietetic Registration.
- Application Basis - Transcript, Preprofessional Practice Experience (completed internship) and CDR (Commission on Dietetic Registration) Exam Results:
- You must submit an official transcript showing your degree as required in Minn. Rule 3250.0010, Subp. 2, Para. A.; sent directly from your university/college to the board office
- Submit a photocopy of the official CDR Exam results received at the test site.
- Submit a photocopy of Verification Statement of completed Dietetic Internship Program.
If you are applying for a Licensed Nutritionist:
- You must submit an official transcript showing your degree as required in Minn. Rule 3250.0020, Subp. 2, Para A.; sent directly from your university/college AND documented proof of completing a preprofessional practice experience component in nutrition practice; OR submit a notarized copy of certification as a Certified Nutrition Specialist by the Certification Board for Nutrition Specialist.
Fees for Licensure:
Licensed Dietitian: Total Due: $282 (Application fee $100.00, Initial License Fee $150.00 and Criminal Background Check Fee $32)
Licensed Nutritionist: Total Due: $357 (Application Fee $175.00. Initial License Fee $150.00 and Criminal Background Check Fee $32)
All fees must be received before an application can be reviewed. ALL FEES ARE NON-REFUNDABLE
Please note: If you have previously held a Minnesota LD or LN you cannot apply for a new license,
YOU MUST APPLY FOR REINSTATEMENT. Please call the board office at 651.201.2764 for information. Fees paid online for an application that requires Reinstatement are non-refundable.
