Disciplinary & Corrective Actions

Conditional License: The individual may practice but must meet certain conditions within a specified timeframe. Conditions may include completing specific coursework, submitting reports to the Board, conducting infection control or recordkeeping inspections, or all of the above. Once all conditions are met, the licensee may petition the Board to remove conditions from the license. If the Board approves, the licensee will be issued an unconditional license.

Limited License: The individual may only practice within a limited scope as defined by the Board. This means they will have fewer responsibilities compared to others who have the same license type. In some cases, the Board lifts the limitation(s) after the licensee meets certain conditions. The Board may also create permanent limitations on a license.

Stipulation to Cease Practice: The individual agrees to cease practice.

Suspended: The individual must cease all practice when the Board suspends their license. As with conditional and limited licenses, the Board may reinstate a suspended license once the individual meets the Board's requirements.

Unconditional License: The license is completely unrestricted by the Board.

Voluntary Surrender and Revocation: The license is removed through an order for voluntary surrender or an order for revocation. In both cases, the individual must cease practicing indefinitely. In order to resume practice, the licensee must comply with the order and apply through the reinstatement process. This action requires Board approval.

Corrective Actions: An Agreement for Corrective Action (ACA) is used 1) to resolve complaints alleging minor violation(s) of the Dental Practice Act and 2) when the nature of the violation(s) does not warrant Disciplinary Action. ACA's are expected to lead to dismissals and neither provide long term monitoring nor impose conditions on licenses. Since ACAs are not disciplinary, they are not published on the Board's website. However, they are considered public information and may be requested by contacting the Board at 612-617-2250 or send an email request to: dental.board@state.mn.us.
