Order duplicate certificates by logging into Online Services.

Multiple Certificates for Clinics

If a clinic is ordering multiple certificates for licensees, complete the Duplicate Certificate Form for each licensee and include payment* in the amount of $10 for each certificate. Have the licensee sign their name and add their email address.

*The board only accepts payment by check or money order (provided they do not say "mobile deposit prohibited"). Cash and credit cards are not accepted.
