    Vaccination Survey for Metro Counties

    The Metro Health & Medical Preparedness Coalition wants to provide support to metro region healthcare professionals who are independent and have no affiliation with hospitals. The metro region includes the following counties- Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington. 

    We have developed a survey that will assist in compiling a list of the identified health professionals that are not affiliated with a hospital.  

    When understanding who is vaccinating healthcare, see the explanation below. 

    1. HOSPITAL HEALTHCARE SYSTEM: If a hospital healthcare system employs you, they will vaccinate you. 
    2. 2. AFFILIATED WITH HOSPITAL HEALTHCARE SYSTEM: If you are affiliated with a healthcare system and provide direct patient care at one of their facilities, that healthcare system may vaccinate you.  Contact that system(s) to verify you are included in their vaccine plan.  This may be the individual you contract with within the system. 
    3. NON-AFFILIATED HEALTHCARE PROVIDER (not connected to a healthcare system): Please fill out the form below.  The information will then be sent to the most appropriate agency to coordinate vaccination planning. 

    IMPORTANT POINTS TO NOTE BEFORE YOU DO THE SURVEY:

    • THEY WOULD PREFER THAT DENTISTS REPORT INFORMATION ON INDIVIDUALS NEEDING VACCINATION AT THE DENTAL CLINIC LEVEL, REPORTING THEMSELVES AND ANY ALLIED DENTAL STAFF IN THE CLINIC (LICENSED OR UNLICENSED) IN THE TOTAL NUMBERS FOR THEIR CLINIC REPORTING. 
    • INDIVIDUAL DENTIST PROVIDERS OR ALLIED DENTAL PROVIDERS PLEASE DO NOT SUBMIT AN INDIVIDUAL FORM UNLESS YOU DO NOT HAVE A CLINIC IN WHICH YOU ARE CURRENTLY WORKING AND ARE LOCATED IN THE ABOVE COUNTIES.
    • IF YOU ARE IN DAKOTA COUNTY AND HAVE ALREADY CONTACTED THE BOARD OF DENTISTRY WITH YOUR CLINIC INFORMATION, PLEASE DO NOT REPORT MORE THAN ONCE, AS THIS HAS ALREADY BEEN COMPLETED. 
    • IF YOU ARE IN ANOKA COUNTY AND HAVE ALREADY EMAILED A FORM IN, YOU DO NOT HAVE TO RESEND. 
    • HENNEPIN COUNTY DECIDED THAT DUE TO AMOUNT OF EMAILS RECEIVED, THEY DO NOT WANT PROVIDERS EMAILING, RATHER THEY CAN USE THIS FORM FOR THEIR REPORTING. 
    • THE COUNTIES/ CITIES WILL CONTACT CLINICS BACK WHEN THEY ARE READY TO BEGIN VACCINATION FOR THEIR AREA.

    SURVEY MUST BE COMPLETED ENTIRELY.

    The information below can assist in understanding the prioritization of who gets vaccinated. Most counties are currently in Phase 1a, first category and may be transitioning into Phase 1a second category soon. Dental offices are currently listed in 1a under the third category.

    Out state county information is also being provided to licensees in their areas, as their counties work to prepare to vaccinate dental providers.
