In accordance with the Minnesota Government Data Practices Act, the Office of Ombuds for Family Child Care Providers is required to inform you of your rights as they pertain to the private information collected from you. The personal information we collect from you is private and any information you provide is voluntary. Your private information will not be shared outside of the OFCCP without your explicit consent. You are not legally required to provide any information. However, should you decide not to provide information, we may not be able to assist you because we will not have the required information.
The government must give individuals notice when collecting private or confidential information from them. This is referred to as a "Tennessen warning notice." Government may also call it a "privacy notice," a "notice of collection of private/confidential data," or something similar. The purpose of the notice is to enable people to make informed decisions about whether to give information about themselves to the government. (See Minnesota Statutes, section 13.04, subdivision 2.)
