    Application Fees

    • Any licensing and processing fees are your responsibility and all fees are nonrefundable. No fee based services can be provided until the full fee is received by the Board office. Fees for online services must be paid with a credit card. Fees paid with a personal check, money order, or cashier's check must be payable to the MN Board of Physical Therapy. A combination of fees may be paid with a single check. Note: NSF penalty will be applied to returned checks. Applications will not be considered complete until all required documents are received by the Board office.
       

      License application by a new PT/PTA graduate $242

      • Application = $100
      • Annual License = $60
      • Criminal Background Check = $32
      • Exam = $50

      License application by a PT/PTA licensed in another state $192

      • Application = $100

      License application by foreign educated PT $192.00-$267.00 (Must apply for licensure by submitting paper application and fees to the Board via mail)

      • Temporary permit fee = $25, required for the 6 month supervised traineeship
      • Exam = $50, required if applicant does not already have a passing score on the exam
      Additional Information
      • The temporary permit request form and fee ($25) cannot be submitted to the Board prior to the receipt of the applicant's initial application and fees.
      • For exam (NPTE) dates and deadlines for MN approval of eligibility for 2026
      • The Exam fee ($50) must be paid for each exam attempt. To retake the exam you must mail the exam fee ($50 - check or money order) to the Board office.
