Licensure Applications and Requirements

Also included in the tutorials is information on studying and scheduling the Minnesota Jurisprudence Examination. 

Before sending your application, please review the CBC policy and complete the pre-application to pay your application fees. Those applying for the Limited Radiology Registration and the Dental Assistant Licensure by Credentials cannot initiate the pre-application, instead you must submit payment (check or money order only) with your mailed application to the Board office.

Credit and debit cards may be used to pay for the application when doing the online pre-application. The Minnesota Board of Dentistry does not accept cash payments. Money orders are only accepted if they do not say "mobile deposit prohibited." We also accept checks. If your application requires proof of education, have your school email your electronic transcripts directly to dental.board@state.mn.us before you send your completed licensure application to our office. Please note that we do not accept transcripts that indicate they have been issued to the student. Acceptable transcripts must list the degree earned and confer date. Our office is currently closed to the public, so all licensure documents must be mailed to the Board's office.
