The Minnesota Board on Aging (MBA) works to ensure that older Minnesotans and their families are effectively served by state and local policies and programs, so they can age well and live well . The MBA does this through its three major roles - administrator, advisor and advocate.
Minnesota Aging & Disability Resources
Minnesota Aging & Disability Resources is a online directory of services and service providers in Minnesota. This new website is currently under construction and soon will have more than 12,000 service providers, offering services at more than 27,000 locations across the state. People will be able to use it to find services in their communities, and professionals will be able to use it to find services for their clients. Be sure to check back soon to find the help you need.
MnCHOICES
MnCHOICES is a computer application used by counties, tribal nations and managed care organizations to support their assessment and support planning work for Minnesotans who need long-term services and supports (LTSS), regardless of age, type of disability or service needs.
Certified assessors, care coordinators and case managers work with people of any age with a disability or anyone in need of long-term services and supports. They use MnCHOICES to discuss your preferences and the supports you need to live a good life. Your responses determine eligibility for a number of public programs, including home and community-based services waivers and Community First Services and Supports. Its rules-based system ensures that everyone is treated equitably. You can learn more by watching a short video about MnCHOICES assessments.
Office of Ombudsman for Long-Term Care
The Office of Ombudsman for Long-Term Care (OOLTC) advocates for older adults in Minnesota who need or receive long-term care. Ombudsmen and volunteers work with older adults, their families, health care and service providers, and public agencies to ensure the health, safety, well-being, and the rights of long-term care consumers. The office also works to improve health care and social services delivery systems via changes in state and federal law and policy. All services provided by the OOLTC are free.

The Disability Hub MN is a free statewide resource network that helps people with disabilities solve problems, navigate the system and plan for the future.
LinkVet
LinkVet, or the Veterans Linkage Line, is a resource for Minnesota veterans and their families, providing access to information and referrals for state and federal veterans benefits, including medical, education, employment and crisis support.
Area agencies on aging
Area agencies on aging (AAAs) are regional organizations that provide the services, supports and information for just about everything that older adults and their families could need. The Minnesota Board on Aging partners with Minnesota’s area agencies on aging to provide Minnesota Aging Pathways services.
Arrowhead Area Agency on Aging
Arrowhead Area Agency on Aging serves northeastern Minnesota, including Aitkin, Carlton, Cook, Itasca, Koochiching, Lake and St. Louis Counties.
Central Minnesota Council on Aging
Central Minnesota Council on Aging serves central Minnesota, including Benton, Cass, Chisago, Crow Wing, Isanti, Kanabec, Mille Lacs, Morrison, Pine, Sherburne, Stearns, Todd, Wadena, and Wright Counties.
Dancing Sky Area Agency on Aging
Dancing Sky Area Agency on Aging serves northwestern Minnesota, including Becker, Beltrami, Clay, Clearwater, Douglas, Grant, Hubbard, Kittson, Lake of the Woods, Mahnomen, Marshall, Norman, Otter Tail, Pennington, Polk, Pope, Red Lake, Roseau, Stevens, Traverse and Wilkin Counties.
Minnesota River Area Agency on Aging
Minnesota River Area Agency on Aging serves southwestern Minnesota, including Big Stone, Blue Earth, Brown, Chippewa, Cottonwood, Faribault, Jackson, Kandiyohi, Lac qui Parle, Le Sueur, Lincoln, Lyon, Martin, McLeod, Meeker, Murray, Nicollet, Nobles, Pipestone, Redwood, Renville, Rock, Sibley, Swift, Waseca, Watonwan and Yellow Medicine Counties.
Southeastern Minnesota Area Agency on Aging
Southeastern Minnesota Area Agency on Aging serves southeastern Minnesota including Dodge, Fillmore, Freeborn, Goodhue, Houston, Mower, Olmsted, Rice, Steele, Wabasha and Winona Counties.
Trellis
Trellis serves the Twin Cities metro area, including Anoka, Carver, Dakota, Hennepin, Ramsey, Scott and Washington Counties.
