Assisted Living Providers

As an assisted living provider, state law requires you to share Minnesota Aging Pathways information with potential residents at the facility tour. Please provide our housing for older adults kiosk card (MBA1050_2024). You can call or chat Minnesota Aging Pathways to request more.

Minnesota Aging Pathways can support people as they make plans and decisions about housing and services. People call 800-333-2433 and press 1 to talk to a specialist and assisted living providers can submit referrals directly to Minnesota Aging Pathways through our referral site.

Effective August 1, 2024: the verification code is no longer required prior to a resident signing a lease. A verification code will not be required or issued for any new or existing residents. Minnesota Aging Pathways will continue to offer support to anyone who chooses to call.
