Types of ID cardsA primary document must include a person's full name and the month, day and year of their birth.
If a person cannot provide one of these documents, they must present proof documents for the type of card they are applying for. A person request an exception if they do not have proper documents for a Standard or REAL ID.
A person will need to apply for a duplicate license if their current ID card is lost, damaged or destroyed. For a duplicate ID card, the photo and signature on file will be used on their replacement card.
A person may also apply for a duplicate ID card if they have changed their name or address. They will need to provide two proofs of their new address if they hold a REAL ID.
A person will apply for a renewal if their card expires in the next 30 days. Our system will default to a renewal nine months before their expiration.
Minnesota law requires that you apply for a new ID card within 30 days of legally changing your name. For a name change, one of the following is necessary:
The document must show the before and after name change. A person must provide documentation of name changes, starting from the name listed on their primary document to their current name.
A person may request an exception if they do not have proper documents to apply for a Standard or REAL ID.
If document is not in English
All documents presented in a language other than English must also have an English translation and a certificate of translation form.

Minnesota driver's license, ID or permit must have a full-face image. A person may be asked to remove eyeglasses.
Religious head coverings are acceptable as long as they do not cover face. Find more information about acceptable head coverings.
Request an exemption if a person has a religious objection with the identification card image requirements.
A person is required to provide their Social Security number on the application form if they are eligible for one.
If they are not eligible for a Social Security number, they must say that on the application form. They may need to supply additional documents.
A Social Security number is not located on or embedded in a Minnesota driver's license, ID or permit. It is private information that is secured on the driver’s license information system and accessible only to those lawfully authorized to access it.See ID card forms and documents
