Copy of the new license period liquor liability insurance certificate (not a declaration of coverage).

License certificate for off sale, wine, and club licenses.

On sale license certificate in the cities of Aurora , Biwabik, Chisholm, Ely, Eveleth, Gilbert, and Virginia, and all county issued on sale licenses also.

Licenses must be executed and signed by local authorities before submitting for Alcohol and Gambling Enforcement Director's signature.

If changes have occurred to the licensee name, this renewal form shall not be used. Rather a full application is then needed. Please call or write to the contact people indicated for proper forms when changes have occurred to the structure or names of individuals involved in the license. Liquor Liability insurance must be in the exact name as the licensee name shown on the preprinted application.
