Forms:
To be qualified to serve as a Nursing Home Administrator (NHA), an individual must complete an approved training course, field experience/practicum, and pass an examination(s) approved by the board. Upon successful completion of the application requirements, a license is issued.
BELTSS utilizes an online service called the “BELTSS Online Portal” to manage individuals contact information, applications, and licenses. All applicants must apply online. You must become a registered user prior to submitting a new application. An individual can only have one BELTSS Online Portal account. If you already have a BELTSS account, then you must use your current account to submit your application. You must pay the applicable fees online before you are considered an active applicant - all fees paid are non-refundable.

Fees:
BELTSS and all other health-related licensing boards receive no general tax funding. Rather, they are completely user funded. Thus the board must charge fees for all services it performs. The fees involved in the licensure process are as follows:
- $150 to open a file with the board and begin the licensure process
- $100 optional fee for review of education and/or experience to determine how many of the requirements for licensure you may already have completed. You may choose to request this service to help you decide whether or not you want to begin the licensure process. This fee is charged only if you seek the determination from the board prior to opening a file. If you seek the determination and then follow through with an official application to begin the licensure process, the $100 fee is applied to the $150 filing fee provided you apply to open a file with the board within one year of paying the $50 review fee.
- $425 to take the NAB exam (This fee must be paid to the test service, not to BELTSS.)
- $100 to take the State exam
- $225 licensure fee to obtain a license once all requirements are complete if initial license is conferred between July 1 and December 31; $100 licensure fee if the license is conferred between January 1 and June 30.
All fees paid to the board are non-refundable.
Another cost associated with meeting licensure requirements is tuition to enroll in courses to satisfy the board’s requirements. The per-credit cost of courses varies from school to school. Consult the program director or course catalog for the schools at which you are considering taking courses.
Period of Issuance: Annual
Length of Determination: Board meets four times a year; national exams are given by appointment; state exams are given every other month.
