
File a Complaint
The Minnesota Board of Cosmetology is dedicated to protecting the public and enforcing the requirements established in Minnesota Statutes Chapter 155A, and Minnesota Rules Chapter 2105 or Chapter 2110. If you believe a practitioner, salon, or school has violated these laws or rules, you may file a complaint with the Board.
The Board does not have the authority to refund money or offer compensation to clients of our licensees. For financial recovery, you may wish to consult an attorney or file a claim with the practitioner's liability insurance.
What Types of Complaints Can the Board Investigate?

- ✅ Infection control or safety violations.
- ✅ Providing services outside the scope of licensure.
- ❌ Customer service disputes.
- ❌ Refund or scheduling disputes.
- ❌ Employment-related disputes.

STEPS TO COMPLETE THE COMPLAINT FORM
1. Previous Complaints: Indicate whether you have submitted a complaint about this issue before.
2. Complainant Section: Provide your name and contact information as the person filing the complaint.
3. Respondent Section: Provide the license number or select "I couldn't find a license number."
4. Statement of Complaint: Clearly describe what happened. Who? What? Where? When?
5. Supporting Documentation: Upload any relevant documents to support your complaint.
6. Authorization: A signed authorization for the release of information is essential. Complaints submitted without this authorization will likely be dismissed.
7. Signature: Sign and submit your complaint.

An investigator will contact you to confirm receipt and may request additional information. The investigation takes time as the Board gathers facts and reviews the case. Once complete, the Complaint Committee will determine what action, if any, is appropriate based on the findings.

Yes, but complaints submitted anonymously or without a signed authorization to release information are likely to be dismissed without investigation. Identifying yourself and authorizing the release of information is an important part of the process.

Yes. Once the Complaint Committee makes a decision and the matter is resolved with the respondent, the investigator will notify you of the outcome. Please note that resolution can take time, even after a decision is made.
