         ▪ Copies of presentation materials including slide shows, handouts, DVDs

         ▪ Course examination and answer key with proportion of questions on each major topic
         ▪ Description of hands-on training evaluation of trainee aptitude for work practices
         ▪ Description of training manager, principal instructor and all other instructor qualifications

    To apply for a training course permit at least 60 calendar days in advance of holding the course.

    Note: This is not for renewals. MN Dept. of Health contact MDH Lead if you need help on how to renew.

    Lead Training Course Notification - Apply Online
    To meet the requirement for training course providers to notify MDH five calendar days before the start of a training course.
    Lead Hazard Reduction Notification - Apply Online
    To notify MDH of lead hazard reduction work at least five calendar days before the project begins. Use this system for initial notifications, amendments, and cancellations. 
    Licensing System Help 
    Answers to common problems had while applying for a credential at the Minnesota Department of Health Licensing System.

    Fees:

    Initial training course permit(each) - $500

    Renewal of an initial training course permit(each) - $250

    Refresher course permit(each) - $250

    Renewal of a refresher course permit(each) - $125

    A cashier's check or money order, payable to the Minnesota Department of Health. Do not send cash. Personal checks are not accepted. This fee is not refundable.


    Period of Issuance:

    A permit is valid for two years from the date of issuance.


    Length of Determination: Allow 60 days for processing and course material review.
