Simply share Minnesota Aging Pathways' housing kiosk card (#MBA1050), which we can mail to you. Just call or chat Minnesota Aging Pathways to request some. Be sure to provide your mailing address and phone number.

Reminder - Getting a verification code before signing a lease is no longer required. However, Minnesota Aging Pathways can support people as they make plans and decisions about housing and services. They can call 800-333-2433 and press 1 to talk to a specialist, and assisted living providers can submit referrals directly to Minnesota Aging Pathways through our referral site.
