Assisted Living Closure Resources

  • MDH Closure Form for Assisted Living Facilities: Assisted living facilities use this form to notify the Minnesota Department of Health, the Ombudsman for Long-Term Care, and the Ombudsman for Mental Health and Developmental Disabilities if an assisted living facility elects to voluntary close.

  • MDH Assisted Living Closure Plan Checklist: This checklist includes the elements that must be present in an assisted living facility’s proposed closure plan.

  • MDH Assisted Living Post-Closure Checklist: This checklist is a useful resource to help assisted living facilities complete all the requirements after approval of a closure plan, including a resident relocation evaluation and resident relocation plan. It describes information that must be sent to a receiving facility and provided to each resident. Before an assisted living facility closes, it must implement the closure plan approved by the Minnesota Department of Health and ensure that arrangements for relocation and continued care that meet each resident’s social, emotional, and health needs are effectuated.

  • AL Notice of Completed Closure: Assisted living facilities use this form to notify the Commissioner that the licensee completed the closure and that the licensee complied with the coordinated move requirements in 144G.55. This form must be submitted after MDH's approval of the closure plan and within 14 calendar days of all residents having left the facility.
