Disability Hub MN is a free statewide service for people with disabilities and the people who support them.

Staff can help answer questions, solve problems and find services. Call 866-333-2466 or use chat at the Disability Hub MN website Monday through Friday, 8:30 a.m. to 5 p.m.

Minnesota Aging Pathways is a free service for older adults and caregivers. It helps people find support and options to live as independently as possible.

Home care provides medical support and help with daily tasks in a person’s home. Services may be short-term after a hospital, nursing home stay or ongoing for people with long-term needs. Home care supports health, safety and daily living while helping people stay where they live.
