Board rules place the following requirements upon all licensees of contact information
Notify the Board within 10 working days of any change in mailing address or phone number. The Board prefers that licensees use a primary practice location for their primary address and phone number, but this is not mandatory. The Board is required to provide an official address for data requests. State Law also requires that you list all practice sites with the Minnesota Board of Optometry. You may go to Online Services or use the form in FAQs to email, fax, or call the board with address, phone number, and email changes.
