Mailing and Drop Off Instructions

The CBCP cannot send fingerprint cards to anyone who does not have an active CBC request from the licensing board. If you have not received the introductory CBC email, we have probably not received a request from the board.

Fingerprints must be submitted to the Criminal Background Check Program (CBCP) on a fingerprint card. We cannot accept fingerprints electronically.

*Please also note that we must receive all original forms signed in ink. No copies or electronic signatures will be accepted.

If you have not received the introductory email, do not contact the CBCP office until you have done the following:

Confirm that your application fees have been received and processed by the board
Confirm your email address with the board
Check your spam and/or junk folders for the introductory email

Holiday Hours

The CBC is closed on State Holidays: New Year's Day, Rev. Dr. Martin Luther King Jr. Day, Presidents' Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Veterans' Day, Thanksgiving and the Friday after, Christmas Day.

We are usually available to take materials Monday - Friday, 8:00am-4:00pm. If staff are unavailable, materials can be left with security personnel at the front desk.

Place forms and FD-258 card in an envelope. It is best if the fingerprint card is not bent.
Write your return address as follows:

        Your Name

        Your Street

        Your City, State, ZIP

        MN Board of (your board) (e.g. MN Board of Nursing)

Send all four forms and the FD-258 card to the CBCP office at the following address:
