Completing the FD-258 Fingerprint Card
Fingerprints submitted to the CBCP on a fingerprint card must be on the official FBI-issued FD-258 card. NOTE: If your fingerprints are taken at the CBCP office, a fingerprint card is not required.

What is the FD-258 Card?
The FD-258 card is the official fingerprint card produced and approved by the FBI. The current version of this card contains the full Privacy Act Statement, as required by the FBI. Older versions of the card, which may mention the Privacy Act of 1974 but do not contain the full section with the heading "Privacy Act Statement," are no longer acceptable.
Why can't I use an equivalent card?
Cards marked as "Equivalent to FD258" do not contain the full Privacy Act Statement on the back of the card. The FBI requires that the Privacy Act Statement be acknowledged at the time you are fingerprinted. By signing the front of the fingerprint card, you are also acknowledging the Privacy Act Statement on the back.
When to use the FD-258 card
If you have your fingerprints taken at the CBCP office, you will not need to submit a fingerprint card.
If you have your fingerprints taken anywhere other than the CBCP office, you must submit them to the CBCP on an official FD-258 card containing the full Privacy Act Statement on the back. See the Sample Fingerprint Cards page.
How do I know if it's the official FD-258 card?
Check the back of the card. The bottom left corner of the card provides information about the card. If you see the words "Equivalent to," it is not an official FD-258 card. If it indicates that it is an official FD-258 card, confirm that the full Privacy Act Statement is displayed on the back of the card. Some older FD-258 cards do not contain the Privacy Act Statement and will not be accepted. See the Sample Fingerprint Cards page.
CBC TIP: Call ahead to the fingerprint location to check whether they have an FD-258 card containing the full Privacy Act Statement on the back. If not, contact the CBCP to request an FD-258 card by mail.
How can I get an official FD-258 card?
Call or email the CBCP and request a fingerprint card by mail. Be sure to include the following information:
- First and Last Name — If leaving a voicemail, be sure to spell your last name.
- Board to which you have applied
- Mailing address where the card should be sent — In some cases, it is helpful to spell street and city names.
